  Complete payment is due at this time, include your credit card information (Visa and MasterCard accepted).
• Step 2: Managers packet: Roster & Liability Release Forms.
  Print out the registration packet (welcome page, sports roster, player waiver release and liability forms). Please bring the completed forms to Capitola Recreation office, 4400 Jade Street, Capitola.
